Office Location

Hallock Road is right off Route 347 in Stony Brook, between Friendly's and the Red Lobster. We are opposite Marshall's, and the Loews Multiplex Cinema. Our office is conveniently located on the ground floor of a clean, well-maintained medical building. Handicap Parking and ramps are located right in front of our door, as well as in our rear parking lot.

Insurance companies frequently change coverage rules and policies. Although we try to obtain accurate coverage information for all of our patients, it is the Patient's Responsibility to obtain any necessary referrals or prior authorizations. If proper referrals are not available at the time of appointment, the patient will be responsible for all charges incurred!!

Special consideration will be given to uninsured patients, patients who have insurance which does not cover some podiatry services, and to patients who wish to see Dr. Smith as an "out of network" provider.
