Insurance companies frequently change coverage rules and policies. Although we try to obtain accurate coverage information for all of our patients, it is the

Patient's Responsibility to obtain any necessary referrals or prior authorizations. If proper referrals are not available at the time of appointment, the patient will be responsible for all charges incurred!!

Some insurance policies have deductibles for medical or podiatry services which must be met before any payments are rendered. It is the responsibility of the patient to meet any deductibles applied by their insurance carrier.

We are constantly reviewing our insurance participation policies so that we can best meet the needs of our patients. If you do not see your insurance carrier listed above, please call our office to see if we can now accept your plan.

Special consideration will be given to uninsured patients, patients who have insurance which does not cover some podiatry services, and to patients who wish to see Dr. Smith as an "out of network" provider.
